Jill Grant, MD
Dr. Jill Grant is an physician with Proactive, MD with the North Lafayette Health Center. Dr. Grant, an Emergency Medicine physician by training, has over 20 years of experience in medicine and a passion for personalized care. She received her medical degree from Chicago Medical School and completed her residency training in Emergency Medicine at the University of Virginia in Richmond. Following residency, she served active duty in the Army and spent 14 months in Iraq during the battle of Fallujah.
Juliana Gaeta, MD
Dr. Juliana Gaeta completed her residency at the Grant Family Medicine Program and earned her Doctor of Medicine from Ross University School of Medicine. Dr. Gaeta holds a bachelor’s degree in science with a minor in Spanish from Purdue University. With nearly two decades of experience, Dr. Gaeta has served in various healthcare settings, including roles at Franciscan Health, Riggs Community Health Center, and St. Francis.
Tina Martinez, FNP-BC
Tina Martinez, FNP-BC, is a dedicated Family Nurse Practitioner with a strong background in both emergency and primary care. She earned her bachelor’s degree in nursing 2016 and began her career on a med/surg unit before transitioning to the emergency department, where she spent six years as an emergency trauma nurse. In 2022, Tina completed her master’s degree in nursing and has since been serving her community in primary care. She has experience in phlebotomy, working in labs, ER, and med/surg.
