Welcome to your new Proactive MD Patient Portal. The online Patient Portal is a central place to manage your health anytime, anywhere. Make appointments, send messages and refill requests to us, and view your test results online.

Make an appointment: You can schedule an appointment online 24/7. You can also view details of upcoming appointments that you’ve already scheduled.

Refill Requests: Request prescription refills online.

Private Messaging: Communicate privately with your provider and our care team any time of the day or night with secure messaging.

Test Results: See your lab results and health history instantly.

Expected Response Time: Inquiries during normal hours should get a same-day response. Inquires made after hours will be answered when the center reopens.

How to Gain Access to the Patient Portal

There are three ways to gain access to the Patient Portal:

  1. Sign up at the Health Center during a scheduled appointment.
  2. Sign up online (instructions below).
  3. Sign up through the Proactive MD Patient Portal app (instructions below).

Instructions for Signing Up Online

1. Follow this link.

2. Fill out all required fields and hit Submit.

3. Click the link in the email from Proactive MD (Please note: this may take up to one business day to arrive in your inbox.).

4. Sign in using your email address in the “User ID” field and the temporary password that was provided in your email in the “Password” field.

5. Verify your date of birth and gender when prompted.

6. Update your password using the temporary password you were provided.

7. Select a security question and answer.

Instructions for Signing Up Using App

1. Search for and download “Proactive MD Portal” app from Apple or Google Play stores.

2. Open the Proactive MD Portal app and click “Don’t have an account? Sign up now.”

3. Fill out all required fields and hit Submit.

4. Click the link in the email from Proactive MD (Please note: this may take up to one business day to arrive in your inbox.).

5. Sign in using your email address in the “User ID” field and the temporary password that was provided in your email in the “Password” field.

6. Verify your date of birth and gender when prompted.

7. Update your password using the temporary password you were provided.

8. Select a security question and answer.

Scheduling Appointments Online

On the toolbar on the left-hand side of any page, if you select Appointments, it will lead you to an Appointment Creator page where you can request one of four types of appointments.

  • Annual Physical
  • Follow Up – This appointment type is used for follow up from a prior Lab Work or Referral Appointment.
  • Lab Work – This appointment type is used when you need to have blood drawn.
  • Sick Visit – This appointment type is used when you want to see a Provider when you are feeling sick and if you want to be tested for a particular illness.

You are able to create an appointment 24 hours out from the time of appointment creation. If you need to get into the Health Center sooner than 24 hours, please call the Health Center at the number listed at the top of any page in your Portal.

  • To create an appointment, you will fill out all 4 criteria listed and select Search.
  • After you click Search, the available appointments will be listed on the right. You will select the Provider and time slot that works for you.
  • Once you select your appointment time and click Submit & Continue, it will direct you to the final page to fill out the reason of your appointment request in the description box. It will also require a contact phone number and email address.

Messaging Your Provider

To message your provider, select Messages from the toolbar located on the left side of the screen, accessible from any page. Here, you can view your conversation history with the care team and exchange private, secure messages with your provider. To send a new message, select New Message and follow the on-screen instructions.
